A new model of remuneration for doctors in the S. João Health Centre in Porto

Payment exercises a powerful influence in workers activity and degree of satisfaction. This is also true for doctors. Scientific evidence about job satisfaction in Family Medicine demonstrates not only an inadequacy of the work regime, model organization and work conditions in the Portuguese Primary Health Care system, when correlated with the professional profile internationally advised, but also a non effective payment system. In our society, physiological needs satisfaction is associated with money. In many cases money can buy satisfaction of social, physiological needs or safety but when individuals are concerned with self-esteem, recognition or scientific updating, money stops being the most appropriate instrument for satisfying their needs. Portuguese family doctors payment system is based on a salary. In 1998, the Ministry of Health implemented a new model of remuneration for Family Doctors in the Portuguese Health Centres through the Law no. 117/98, of May 5, designated by Experimental Remunerating System for General Practice Doctors. Also in 1998 a collaboration agreement was celebrated between the North Regional Health Administration and Portos University of Medicine. This project was called Projecto Tubo de Ensaio/ São João Health Centre, being one of their main objectives to Develop Original Projects in the extent of the Health Administration, Health Administration Services and Health Care. One of these projects, designated by Experimentation of a new payment model for doctors consists on linking remuneration with production, considering capitation and incentives to rational prescription of medicines, exams and therapeutics.Downloads
