Medical error: Patient safety - A health quality indicator

This paper reviews the history marks of patients safety, defines error taxonomy and accident mechanisms by puting emphasis on system theories, and safety culture development. Team work and learning opportunities around errors are of utmost importance to improove patient safety, as well as the louching of a real just culture, where error repport and disclosure are encouraged in a blame free, but responsible atmosphere.
