Limitations and constraints of various natures have been identified to the heath system including low accessibility, duplication of care, absence of communication between primary and secondary care, and lack of continuous and global care. In spite of the well known limitations of the system, institutional, cultural, political and economical factors represent significant barriers to the changing process, creating a gap between hospitals and health centers. In this paper, the author suggests a stepping process aiming to establish communication and cooperation between primary and secondary care as a way to create a patient-centered health system.Downloads
